| ON THE WEB. Log into the Campus Information System and click on your address, telephone number, or email address to update the information.

DIPLOMA MAILING ADDRESS. The Graduation Division requires a signature to change your diploma mailing address. Requests to update/change diploma mailing addresses cannot be made by phone. Diploma mailing address changes can be made:
- In person. Go to 250-N Student Services Building, window 15.
- By mail. Send a written request (including student ID number, new address, and signature) to: Graduation Division, 201 S 1460 E Room 250 N, Salt Lake City, UT, 84112
- By fax. Fax your request (including student ID number, new address, and signature) to (801) 585-7860.
Please be aware that changing your University mailing address will not automatically change your diploma mailing address.
