SYSTEM NAME: The area in which the web function you are using lives, for example, Changing your email is in both the Student and Employee Systems. If you are from the Registrar's Office calling about the Student system the System Name: Student
PROGRAM NAME or WEB ADDRESS: This is the web page where you are seeing the problem happen, cut and paste the URL into the box. For example, as in the system name you are reporting a problem on the student side with ChangeEmail, Program Name: Change Email or you can cut and paste the URL http://www.acs.utah.edu/bin/student/ChangeEmail.... into the box.
